Job Responsibilities :

Under the supervision and direction of a licensed nurse, responsibilities of this position include, but are not limited to :

  • Performs activities of daily living for residents such as bathing, oral care, grooming, dressing, toileting, feeding, walking, exercising and encouraging independence
  • Observes patients' conditions, measuring and recording food and liquid intake and output and vital signs
  • Transfers residents via wheelchairs and performs patient transfers in accordance with plan of care (i.e. sit to stand and hoyers)
  • Passes medications, if able to provide documentation of completed medication training

  • Educational Requirements :
  • Must hold a current Certified Nursing Aide Certification in the state of Wisconsin
  • Previous experience in a long-term care facility preferred, but not required
  • Must be able to express self in written and oral communication and communicate effectively in care setting with residents, families and staff members
  • Ability to maintain confidentiality
  • Ability to work independently or with a team
  • Conducts oneself in a professional, caring, sensitive and safe manner which promotes positive relations and rapport with residents and visitors
