Position Summary
The Community Support Case Manager works with families referred by the Philadelphia Department of Public Health Phillies Families CAN program, to help them achieve goals including community resource linkages and overall family stability. This position conducts thorough assessments of the strengths and needs of families, develops a service plan, implements problem-solving interventions, and coordinates warm handoffs to community programs/services. The Community Support Case Manager guides families through the process of enrollment in supportive programs using a strengths-based approach and respecting the family’s self-determination. This position supports families through advocacy, provides essential resources, and enhances the protective factors of families at-risk of child welfare involvement. The Community Support Case Manager conducts home visits, maintains continuous communication efforts with families, and connects families to resources that help improve overall well-being.
This position reports to the FEC Program Manager. This is a HYBRID position.

Essential Functions

- Provides in-home services to at risk families or individuals as determined by the Public Health Phillies Families CAN program referral.
- Provides case management services including but not limited to home visits, family assessments, crisis intervention, advocation, resource connection, and case file maintenance according to program standards as needed.
- Supports with other Tier I cases within the FEC program, as needed.
- Collaborates with service providers as needed to communicate progress and work toward successfully connecting families to resources.
- Safeguards the confidentiality of client families.
- Promptly reports all conflicts among client families, or serious incidents involving client and families within 24 hours of the incident.
- Provides services and support outside of normal working hours to accommodate the family’s needs, as necessary.

- Ability to understand, acknowledge, and assess client needs to provide a timely, trauma informed approach in service.
- Ensures the appropriate handling of sensitive information and understanding for issues such as confidentiality, domestic abuse, child abuse, and mandated reporter requirements.

- Attends all Congreso trainings and in-services as required, including relevant professional groups to maintain professional, requisite knowledge and licensure.
- Attends, participates, and facilitates relevant staff meetings to promote communication and execution of goals.
- Ability to effectively use standard office equipment, including a variety of common software programs such as Office365, UNIDAD, and other customized databases.
- Follows through on assignments, works in a timely manner, within established deadlines, and completes special projects specific to the function of the department, as needed or as directed by supervisor.
- Demonstrates ability to work independently and collaboratively with diverse groups, in a compassionate, courteous, and respectful manner that fosters an inclusive environment.
- Strong written and verbal communication skills including effective and respectful communication with individuals and groups.
- Bilingual (Spanish/English) written and verbal skills is a plus.
Supervisory Responsibilities:
This position has no supervisory responsibilities.
Minimum Experience, Education, and Qualifications
Minimum Education:
Bachelor’s degree in social work or related field, required.
Minimum Experience:
1-3 years’ related experience, a plus.
Familiarity with the child welfare system and trauma informed training a plus.
Certification/License:
Mandated Reporter training certificate and VRA Supporting Children & Families with Disabilities are required and may be completed post hire.
