Key Responsibilities:
Provide skilled nursing care to home health patients in the Jackson area, including assessments, treatments, and ongoing evaluation of patient progress.
Develop and implement patient care plans in collaboration with physicians, family members, and other healthcare professionals.
Manage a caseload of 25-35 patients per week, ensuring high-quality care and timely visits.

Qualifications:
