- Schedule: Night Shift (3x12s per week)
- Weekends: Every third
- Employment Type: Full-Time, Permanent
- Start Date: ASAP
As an LPN in a Skilled Nursing Facility, you will play a crucial role in providing exceptional patient care to residents requiring short-term rehabilitation or long-term skilled nursing services. You will work closely with registered nurses (RNs), physicians, and other healthcare professionals to ensure the highest level of care and comfort for patients.
Key Responsibilities:
- Deliver high-quality nursing care in accordance with SNF protocols and patient care plans.
- Administer medications and treatments as prescribed by healthcare providers.
- Monitor and document vital signs, symptoms, and patient progress.
- Assist with wound care, IV therapy, and other skilled nursing procedures.
- Provide emotional and physical support to residents and their families.
- Collaborate with interdisciplinary teams to create and implement individualized care plans.
- Ensure compliance with state and federal regulations, as well as infection control protocols.
- Respond to medical emergencies and implement appropriate interventions.
- Active LPN license in the state of Ohio (or compact license with eligibility to practice in OH).
- Certifications Required: Basic Life Support (BLS).
- Preferred Certifications: IV Certification, Wound Care Certification (WCC).
- Minimum Experience: 1 year of experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting preferred.
- Strong communication, problem-solving, and patient care skills.
