Job Description : Provide exceptional physical therapy services to patients in a home health setting.Develop personalized treatment plans based on patient assessments and goals.Conduct thorough patient evaluations and progress assessments.Collaborate with interdisciplinary teams to optimize patient care outcomes.Educate patients and caregivers on therapeutic exercises and home care techniques.Education and Certification Requirements : Valid Physical Therapy license in New Mexico.Minimum of 2 years of experience, with home health experience preferred.
