Connected Health Care, a premier recruitment and staffing firm, is seeking a Local Travel/Contract Licensed Practical Nurse (LPN) for a skilled nursing facility (SNF) near Warren, MN

Job Description:

• Provide skilled nursing care and support to residents in a long-term care (LTC) or skilled nursing facility (SNF) setting.
• Administer medications, perform wound care, and assist with daily living activities under the direction of registered nurses and physicians.
• Monitor and document patient health status, progress, and response to treatment.
• Collaborate with healthcare teams to ensure comprehensive and compassionate patient care.
• Adhere to facility policies, procedures, and regulatory guidelines.
  
  

Education And Certification Requirements:

• LPN license.
• Completion of an accredited practical nursing program.
• Minimum of 1 year of experience in long-term care (LTC), skilled nursing, or rehabilitation settings.
