We are unique in that we are a provider of long term care services. While many of our long term residents receive services under Medicare Part A and B, we do not accept short term admissions. DUTIES AND RESPONSIBILITIES

Responsibilities of the MDS Coordinator - (RN) Registered Nurse:

As the MDS Coordinator you will collaborate with members of our health care team to assist our residents in achieving established outcomes of care. In this key role, you will be responsible for coordinating and overseeing the MDS (Minimum Data Set) process to ensure accurate and timely assessments of residents. You will work closely with our interdisciplinary team, including our Medical Director, APRN, therapists, and social worker, to develop individualized plans of care.

Key Responsibilities:

  • Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of RAPs and Triggers. Using Point Click Care you will be responsible for electronically transmitting all data to the State in accordance with HCFA and State of Connecticut laws.
  • Develop preliminary and comprehensive MDS assessments of the nursing needs of each resident
  • Adhere to a timetable of MDS assessments required for our Medicare residents.
  • Completes assessments, Minimum Data Set (MDS) and care plans for 60 long term residents
  • Monitors completion of MDS’s by other disciplines within timeframes prescribed by regulatory guidelines
  • Monitor and track resident progress to ensure quality care is delivered and documentation is maintained.
  • Provide education and guidance to staff regarding MDS processes and documentation standards.
  • Advises supervisor of incomplete and/or untimely assessments by disciplines other than nursing.
  • Initiates care plans and supporting activities that will result in the best possible outcome for assigned residents.
  • Generates and distributes monthly care plan calendar for the following month.

· Participate in care conferences, quality assurance meetings, and other team-based activities.

· Review and update resident data as needed, ensuring that all required information is properly documented.

· Monitor for changes in residents’ conditions and communicate necessary adjustments to care plans.

  • Gather data and complete Resident Assessment Instrument (RAI) in an accurate and timely manner.
  • Effectively communicate with the DNS of any issues or concerns regarding completion of the MDS/Care plan and concerns regarding direct resident care.
  • Complete Medicare appeals
  • Manage Myers and Stauffer reviews
  • Ability to receive consultation from reimbursement consultant

EXPERIENCE

· Previous experience with MDS assessments and care planning is required. In-depth knowledge of federal and state regulations governing skilled nursing facilities.

· Strong attention to detail, organizational skills, and ability to prioritize tasks.

· Excellent communication skills, both verbal and written.

SKILLS AND COMPETENCIES

  • Strong clinical assessment skills.
  • The ability to make independent decisions regularly.
  • Effective interpersonal skills, enabling you to collaborate with an interdisciplinary team, residents and families
  • Knowledge of Point Click Care preferred
  • Exceptional written and oral communication skills.
  • Proven decision-making and analytical abilities.
  • Ability to read, write, speak, and understand the English language clearly
  • Must be able to relate to older people in a dignified manner and build a relationship with residents that are positive and supportive.
  • Working knowledge of computers and ability to learn and utilize electronic documentation.
  • Must possess leadership and supervisory ability with strong teamwork willingness and aptitude.

LICENSE

Valid State of Connecticut license as Registered Nurse.
