Job Description
The Building for Health Program Manager is responsible for planning, executing, and evaluating the implementation of Building for Health : a cross-sector initiative designed to strengthen communication and coordination among various programs and services that aim to enhance the livability of homes and promote the well-being of residents. Building for Health serves as a centralized hub, streamlining the referrals of a range of services, including but not limited to : home hazard mitigation, energy efficiency upgrades, financial and legal assistance for housing matters. The Program Manager will be responsible to manage the implementation of Building for Health's core components such as the shared and centralized infrastructure, the partners, identification and engagement, community representation for decision making, and Data Collection and Analysis
Responsibilities
Shared and Centralized Infrastructure :
- Track and manage referrals between partners, ensuring timely follow-up and appropriate connections for residents.
- Maintain and refine tracking tools and workflows to document referral processes and enhance communication between partners.
- Conduct follow-up for specific referrals requiring additional outreach to confirm residents have been connected to services.
- Support the Connecticut Children's Healthy Homes team with referrals as needed.
Partner Engagement :
Identify, recruit, and engage partners aligned with Building for Health's scope and mission.Lead onboarding and training of partners on communication, needs assessment, and referral management.Facilitate regular partner engagement meetings and ensure continuous feedback loops are maintained.Supervise partner subcontractsAssess partner satisfaction and identify opportunities for improved engagement and collaboration.Community Engagement :
Identify and recruit residents with lived experience from the target community to inform the initiative.Develop strategies to incorporate resident input into decision-making processes to refine the Building for Health model, including conducting community engagement meetings.Data Collection and Analysis :
Support the development and execution of data collection methods to assess Building for Health's impactMonitor program progress and recommend improvements for each core component.Track outcome metrics and provide reports on program impact to partners, funders, and other stakeholders.Assist in the analysis of program sustainability and scalability.Conduct reports of progress and impact to different audiences, including partners, community members, CT Children's, and funders.Project Management :
Lead project management efforts, including work plan development, timelines, performance measurement, budget oversight, invoicing, and partner and community incentivesSupport change management, quality improvement, and capacity-building activities.Building and Cultivating Relationships :
Cultivate relationships with internal teams and external partners to ensure alignment with project goals.Demonstrate commitment to diversity, equity, and inclusion in all program activities.Demonstrate support for the mission, values, and goals of the organization through behaviors that are consistent with Connecticut Children's standards.Performs other job-related duties as assigned.
Qualifications
Education and / or Experience Required :
Bachelor's Degree in public health, healthcare, business or public administration.Minimum of five (5) years of experience in public health, healthcare, business or public administration.Demonstrated leadership experience.Education and / or Experience Preferred :
Master's Degree in public health, healthcare, business or public administration.With Master's degree, three (3) years of experience in public health, healthcare, business or public administration.Lived experience in an urban areaLicense and / or Certifications Required :
