Job Overview : An Internal Medical Group has an opportunity for a Physician to work in Skilled Nursing Facilities in the Avon area. The position would be full time working 40 hours per week.
Summary :
Under indirect supervision, provides and manages health care for a specified patient population.
Essential Functions :
- Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
- Engages patients and their families or caregivers in understanding, setting and monitoring self-management care plans, and documents individualized care plans and care coordination in the patient chart.
- Analyze patients' medical history, medication allergies, physical condition and examination to determine best care.
- Diagnose medical conditions.
- Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
- Refers patients to specialists or to internal medicine as appropriate.
- Directs and coordinates the patient care activities of nursing and support staff as required.
- Respects and promotes patient rights and privacy.
- Responds appropriately to emergency situations.
- Participates in meetings to evaluate practice processes and workflows pertaining to patient care and quality improvement efforts.
- Participates in specified health promotion, education and / or prevention programs.
- Participates in population health activities as directed by the practice.
- Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and / or infection control standards.
