Pay Range : $200,000.00 to $300,000.00 per year

Job Overview : An Internal Medical Group has an opportunity for a Physician to work in Skilled Nursing Facilities in the Avon area. This is an excellent opportunity for those that enjoy a flexible work schedule, working autonomously, and thrive in a pay-for-performance environment. The position would be full time working 40 hours per week.

Summary :

Under indirect supervision, provides and manages health care for a specified patient population.

Essential Functions :

  • Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
  • Engages patients and their families or caregivers in understanding, setting and monitoring self-management care plans, and documents individualized care plans and care coordination in the patient chart.
  • Analyze patients' medical history, medication allergies, physical condition and examination to determine best care.
  • Diagnose medical conditions.
  • Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
  • Refers patients to specialists or to internal medicine as appropriate.
  • Directs and coordinates the patient care activities of nursing and support staff as required.
  • Respects and promotes patient rights and privacy.
  • Responds appropriately to emergency situations.
  • Participates in meetings to evaluate practice processes and workflows pertaining to patient care and quality improvement efforts.
  • Participates in specified health promotion, education and / or prevention programs.
  • Participates in population health activities as directed by the practice.
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and / or infection control standards.

Skills and Knowledge :

  • Knowledge of legal and ethical standards for the delivery of medical care.
  • Understanding of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosage and instruct patients in correct usage.
  • Knowledge of current principles, methods and procedures for the delivery of medical evaluations, diagnosis and treatments in area of expertise.
  • Demonstrated skill in interviewing and patient coaching.
  • EHR documentation as it pertains to patients and populations of patients.
  • Education and Experience :

  • Medical Doctor with 3 to 5 years of direct patient experience which may include residency.
  • Medical License, Board certification, Federal DEA certification.
  • Need to be Board eligible or Board Certified.
