Social Worker (part-time)

We are a care home placement agency working with various Managed Care Plans as a Community Support (CS) Provider for the California Advancing and Innovating Medi-Cal (CalAIM) program. CalAIM is similar to another existing Medicaid Waiver Program, the Assisted Living Waiver (ALW), which allows Medi-Cal to pay for residential care facilities for the elderly (RCFEs): either small 4-6 bed residential homes or large 100 bed assisted living homes for qualified individuals.

To minimize travel time, we are looking for positive and outgoing applicants to cover 1 of 4 regions in each county (North, South, East, West). In your cover sheet please indicate which region you would prefer to cover. Also, if your schedule is flexible to permit occasional in-person hospital in-services, please indicate that as well.

The CS CalAIM social worker position has three parts:

1) Monthly in-person quality check visits to CS members placed at CS RCFEs and to input the completed quality check assessment tool into our online database. For each monthly visit we will pay $45 for each client visit plus $15 for gas expenses (e.g., a total of $60 for each client visit).

2) One time visits to RCFEs to assess and evaluate each home for CS RCFE network inclusion . For each RCFE visit we will pay a flat fee of $40. We will work with the CalAIM social worker to determine which RCFEs to visit based on licensing reports.

3) Occasional in-person hospital, SNF, etc. CalAIM CS presentations (optional). For each presentation the pay is $105 ($45 for 2 hours (includes travel time) $15 gas).

The job is very flexible and can even be done on weekends. The program is new and the volume of members visited each month will increase through outreach and referrals received through primary care provides, community-based organizations and the managed care plan.

Pay:

RCFE Monthly Visits to Clients:

1) $45 $15 gas (for a total of $60) for each monthly client visit for quality check assessments at any RCFE in the CS RCFE network.

Initial RCFE Network Set-Up:

2) $40 for each RCFE visit to RCFEs for the initial RCFE CS Network set-up.

Occasional Hospital In-Person In-Services (Optional)

3) $90 $15 gas for occasional in-person hospital in-services (optional).

Job Requirements:

The social worker must have a Master's degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology plus one year of related work experience.

The job applicant must:

Have own car and transportation.

Have own computer or laptop with internet connection.

Be familiar with computers and data entry
