Position Summary:
Connections That Matter, Inc’s (CTM’s) Psychiatric Rehabilitation Program provides community-based comprehensive rehabilitation and recovery services and support. The program strives to assist individuals with successful community integration, along with the use of community resources. CTM’s rehabilitation services assist children, adolescents, and adults with developing and utilizing community living skills, completing activities of daily living, and strengthening family and peer support. Services are designed to meet the specific rehabilitation needs of clients and can be provided in-home, in-office, in-school or other approved/appropriate settings.
Responsibilities:
- Collaborate with parents, therapists and other treatment team members to address mental health needs and behavioral concerns.
- Assist the program participant with following an Individual Rehabilitation Plan (IRP) developed by a licensed mental health professional.
- Complete documentation for each day of service provided using the company’s Electronic Medical Records (EHR).
- Participate in scheduled supervision and monthly mandated in-service training.
Qualifications:
- High School Diploma or GED (Required)
- An Associate’s (or equivalent college credits from a four-year college) or Bachelor’s Degree in social work, psychology, mental health or another related human services field.
- At least one (1) year of previous experience working with young/adults in a mental health setting.
- Experience using computer software.
- Experience in the mental health or human services field.
- Able to manage a flexible but committed schedule.
- Able to pass a criminal background check.
Required License/Certification:
- Driver’s License
- Proof of current Auto Registration, Inspection and Insurance
