Summary of Job Description:

The case manager will work with clients of different age ranges and backgrounds, persistent mental health concerns, and complex needs. The goal of the program is to provide responsive and comprehensive services to clients to improve their stability, wellness, and recovery. The case manager will assist in making referrals, coordinating care, and work with multiple systems including but not limited to jails, hospitals, vocational and housing programs, schools, legal systems, and medical providers.

This position will provide services to 15-24 year old young adult harmed by the criminal justice system living in Seattle/King County. Services will be provided at home, school, community, detention centers and other facilities where client services are needed. System navigation services for this population may include but it is not limited to: Criminal justice involvement, educational needs, housing needs, health services (both physical and behavioral health services), employment training, vocational rehabilitation, emergency response, and child welfare and finance management. It is expected that client enrolled in the program will receive trauma informed care and motivational interviewing strategies.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • The case manager will work in partnership with the client and other providers in an effort to provide responsive, linguistically and culturally sensitive, and comprehensive services.
  • Case Manager will work primarily with youth, ages 15-24, and their families from different backgrounds, who have persistent high-risk behaviors, with complex needs. However, from time to time we may serve clients who are younger or older depending on the referral and the need of the clients.
  • Case Manager through the newly developed Youth at Risk -Assistance, Mentoring, Outreach & Re-entry Program will offer services to youth/young adults who are currently incarcerating and/or are in the process of transitioning back to the community.
  • Maintain a caseload of 35 active clients at all times
  • Provide direct services daily. These services may include, case management, individual services, group services, phone services, care coordination, etc.
  • All services must be properly documented at Consejo records.
  • Conduct outreach and engagement efforts
  • Develop marketing materials to reach out the target population.
  • Provide advocacy, outreach, information and referral services to low income clients in need of case management services including wrap around services, as needed.
  • Serve clients with criminal justice involvement, gang involvement, etc.
  • Ensure that clients in need of SUD and MH services are connected to Consejo treatment services.
  • Coordinate services with other internal departments (mental health, domestic violence, substance abuse, etc.), other service providers and/or agencies to ensure optimal client care.
  • Provide youth and their families with communication skills, tools and other resources to support the recovery of the client.
  • Provide early intervention & education; assistance to overcome barriers in transitioning out; mentoring opportunities: coordination guidance; re-entry plan development: youth transitioning out of detention center.
  • Assess crises and provide crisis intervention when necessary.
  • Establish liaisons to access community services, e.g. medical care, housing, food, vocational programs, to include GED resources, etc.
  • Provide education, outreach and information in the community as needed, including services in the Juvenile detention facilities.
  • Promote healthy living techniques for families to model/share within their communities
  • Serve as a liaison between the client and the multiple numbers of systems utilized by the client.
  • Advocate on behalf of the client to ensure that the client remains in school, home, and in the community when appropriate.
  • Work in partnership with law enforcement, probation department, department of corrections, and judicial system to reduce the risk of re-offense or continue criminal justice involvement.
  • Create an individualized plan for each client to monitor their progress and service success.
  • Use all the tools available to you to keep client engage in services.
  • Reduce the client's law enforcement involvement, truancy, school suspension, juvenile detention/jail, psychiatric hospitalization, out-of-home placement, and homelessness.
  • Complete all required documentation in order to be 100% file compliant.
  • Attend mandatory clinical in-services/trainings, supervision, and regularly scheduled department/agency meetings.
  • Fulfill other duties as may be assigned to meet agency operation's needs.

Recovery Best Practices

  • Ability to understand and apply the ten core principles from the national consensus statement on recovery (hope, self-direction, individualized and person-centered, empowerment, holistic, non-linear, strengths-based, peer support, respect, and responsibility).
  • Knowledge on conducting strengths-based interviews and how to utilize strengths in implementing the care plan.
  • Provide outpatient evaluations, assessments, care plans and progress notes that are individualized and recovery oriented.
  • Ability to assist clients to assume or resume employment, education, or normalized housing, and to thrive in these roles.
  • Ability to locate community resources outside of the mental health system and inspire consumers to use them.
  • Provide culturally competent recovery practices for the Latino clients.
  • Knowledge on how to implement a Wellness Recovery Action Plan.
  • Knowledge on motivational interviewing skills to enhance recovery potential.
  • Skills in helping people who have experienced trauma to recover.
  • Provide family psycho-education and engagement.

QUALIFICATIONS:

  • Bachelor's degree with major specialization in criminal justice, social, behavioral or human services related fields or High school diploma or GED with related volunteer experience comparable to two years full time work in a social, behavioral, or human services or related fields.
  • The employee is required to have a valid WA state driver's license at the time of employment, able to safely operate a motor vehicle and have reliable transportation as this position is required to travel within the vicinity of the community in which Consejo services.
  • Proficient in speaking, reading and writing Spanish and English
  • You must be eligible to work in the United States of America and provide documents that support eligibility.
  • You must sign an oath of confidentiality covering all patient-related information and will be the designated agency privacy officer. You will ensure 42 CFR and HIPAA confidentiality requirements.
  • You must pass a Washington State criminal background check and pass two federal fraud and abuse exclusion checks.
  • You must obtain an agency affiliation counselor certification.

WORK SCHEDULE

This full-time in-person position works a typical 8 hours a day between 8 AM - 5 PM, Monday to Friday.

SALARY RANGE

$25.00 - $27.88/hr. depending on experience, qualifications and credentials.
