The Housing and Employment Case Manager will work with and support clients to seek and obtain gainful employment and housing within the community. The position may require the case manager to provide training and preparation needed for establishing ADL’s and pre-employment.

Responsibilities and Duties:

Primary duties include but are not limited to the following:

· Communication with all referral sources including community partners.

· Maintaining knowledge of program services and implementation procedures

· Works with client to identify opportunities based on clients’ needs.

· Assists clients with employment applications and other supportive paperwork.

· Prepare client for job interviews.

· Ensures client confidentiality per HIPAA guidelines, including the use of secure email.

· Work with clients to develop individualized employment plan as outlined by program specifications.

· Adheres to strict boundaries and professional ethics in the care of others.

· Continually survey the job market for availability with emphasis on client’s goals and objectives.

· Maintain Client Records and case notes in alignment with company protocols.

· Maintain accurate documentation of service objectives and outcomes

· Conduct client visits per contract guidelines.

· Continually monitor and evaluate each client’s progression through employment case management plan and develop revisions to the plan as needed.

· Attend program-related community, coalition and committee meetings as assigned.

· Conducts crisis intervention as needed.

· Case conferencing with other service providers as needed to support clients.

· Utilizes emergency call list as needed.

Qualifications:

· Education and experience vary by location and program.

· Preferred is a 4-year degree in human services or comparable combination of education/work related experience.

o Experience in social service setting with working knowledge of case management systems and planning techniques.

o Knowledge of employment related resources.

o Basic familiarity with mental health and substance abuse disorders.

· Must be proficient in the following computer applications—Microsoft Office Programs and other web-based platforms.

· Must have own transportation to conduct job related travel, ability to drive and meet insurance carrier’s requirements for approval to drive

Physical Requirements:

· On an occasional basis, may be required to remain in stationary position >50% of the day.

· Sit/Stand/Walk in combination inside the office to access file cabinets, office machinery, etc.

· Sit/Stand/Walk in combination outside the office to access vehicles, client houses, hospitals, etc.

· Constant use of upper extremities for Computer, office machinery and driving.

· Frequent communication via phone, fax, email and in person for coordination of care, outreach, etc.

· Must be able to observe and assess clients in their personal living space.

· May lift up to 25lbs on a frequent basis.

· Outdoor, local travel and driving is part of the role, therefore, weather exposure is a given.

· Due to the vulnerability of the population, we serve, COVID-19 vaccination is required.

Mental Requirements:

For an employee to succeed in this position they will need to be able to have time management skills, adapt to changing schedules, and contribute to problem solving when needed.

Job Type: Full-time

Pay: $21.00 - $23.00 per hour
