The Housing and employment Case Manager will work with and support clients to seek and obtain permanent housing and/or gainful employment within the community. The position may require specialist to provide training and preparation needed for establishing ADL’s, as well as pre-employment training and preparation. This role is based out of our company office

Responsibilities and Duties:

  • Communication with all referral sources including community partners.
  • Maintaining knowledge of FCS services and implementation procedures
  • Works with client to identify opportunities based on clients’ needs.
  • Assist client with the application process.
  • Prepare client for job interviews.
  • Coordinate and develop all documentation utilized in the delivery of FCS housing & employment services.
  • Ensures client confidentiality per HIPAA guidelines.
  • Work with clients to develop individualized housing & employment plans.
  • Adheres to strict boundaries and professional ethics in the care of others.
  • Continually surveys job market for availability with emphasis on clients goals and objectives.
  • Maintain Client Records and case notes
  • Assists clients with housing applications, completes supportive and subsidized housing paperwork.
  • Maintain Client Records and case notes
  • Maintain accurate documentation of service objectives and outcomes
  • Ensure client Follow-up takes place.
  • Continually monitor and evaluate each client’s progression through housing & employment case management plan and develop corrective action revisions to the plan as needed.
  • Attend program-related community, coalition and committee meetings as assigned.
  • Conducts crisis intervention as needed.
  • Case conferencing as needed
  • Ensure effective service delivery by coordinating work with other case managers assigned to client.

Qualifications:

  • 4-year degree in human services or comparable combination of education/work related experience preferred
  • Experience in social service setting with working knowledge of case management systems and planning techniques;
  • Knowledge of housing related resources both temporary and permanent.
  • Knowledge of employment related resources.
  • Experience working with homeless individuals and families a plus;
  • Must be proficient in the following computer applications--Microsoft Word and Excel.
  • Must meet insurance carrier’s requirements for approval to drive.
  • Must have own transportation to conduct job related travel and must be able to drive

Physical Requirements:

  • On an occasional basis, may be required to remain in stationary position >50% of the day.
  • Sit/Stand/Walk in combination inside the office to access file cabinets, office machinery, etc.
  • Sit/Stand/Walk in combination outside the office to access vehicles, client houses, hospitals, etc.
  • Constant use of upper extremities for Computer, office machinery and driving.
  • Frequent communication via phone, fax, email and in person for coordination of care, outreach, etc.
  • Must be able to observe and assess clients in their personal living space.
  • May lift up to 20lbs on a seldom basis.
  • Outdoor, local travel and driving is part of the role, therefore, weather exposure is a given.

For an employee to succeed in this position they will need to have time management skills, adapt to changing schedules, and contribute to problem solving when needed.

Job Type: Full-time

Pay: $21.00 - $23.00 per hour

Expected hours: 40 per week

Schedule:

  • 8 hour shift
  • Day shift
  • Monday to Friday
  • No weekends

Experience:

  • Case management: 1 year (Preferred)
  • Supportive Housing: 1 year (Preferred)

License/Certification:

  • Driver's License (Required)
