Constant Care Health Services is currently seeking highly trained Licensed Practical Nurses (LPN) for our 5 Star Skilled Nursing Facilities located in The Bronx. Our facilities are committed to providing high quality services and to do so we need amazing Licensed Practical Nurses (LPN) .

RESPONSIBILITIES:
- Make daily round to observe residents and nursing care
- Provide Bedside Care
- Administer medications and document
- Observes and reports patients’ vital signs, symptoms and general condition to the appropriate individuals(s)
- Performs other nursing functions related to patient care and nursing service, as delegated by the Nursing Service Administration.
- Licensed Practical Nurses under the supervision of a registered nurse provides medical care, including treatments, administering medication following the direct plan of care for each of our residents
QUALIFICATIONS:
- Must possess and maintain a current NYS state license as a Licensed Practical Nurse (LPN)
- Preferred to have experience in a Long-Term Care environment, but new grads are welcome!
- Must Possess BLS
