Home Health Aid

Job Overview
We are seeking compassionate people to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the support they need to maintain their health and well-being. You will work closely with patients, families, and interdisciplinary teams to develop and implement personalized care plans that address the unique needs of each individual.

Responsibilities

• Assist patients with activities of daily living, including personal hygiene, meal preparation, and laundry.
• Provide catheter care and monitor patients for any changes in condition.
• Support individuals with memory care needs and those with disabilities by implementing appropriate interventions.
• Maintain accurate patient records and documentation in compliance with healthcare regulations.
• Collaborate with healthcare professionals to coordinate comprehensive home community care services.
• Educate patients and families on health management strategies and self-care techniques.
• Ensure a clean and safe environment for patients by adhering to infection control practices.

Experience

• Experience in home health or senior care is preferred but not required.
• Familiarity with systems used for patient management and documentation is an asset.
• Strong interpersonal skills to effectively communicate with patients, families, and team members.
• Ability to work independently while managing multiple priorities in a home setting.

Job Types: Full-time, Part-time, PRN

Pay: $14.00 - $15.00 per hour

Expected hours: 20 – 40 per week
