Admissions Registered Nurse (RN) Home Health
Summary:
Constellation Health Services is seeking an Admission RN for our home health program. This Registered Nurse (RN) will be primarily responsible for performing the admission visit for home health patients referred to service.
This is a field-based position.
Responsibilities:
- The primary responsibility for this role will be to do admission visits as assigned with follow up visits as needed.
- Provide skilled nursing and head-to-toe assessments to our patients.
- This includes the oversight of medication administration and wound care treatment as prescribed in the physician's plan of care.
- Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
- Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system.
- Submits all clinical documentation, time records and other information in the required time frame.

Qualifications:
- Licensed Registered Nurse (RN) in Connecticut with at least 1 year of clinical experience.
- A graduate of an accredited RN program.
- The employee has the required licensure, education, training, and qualifications required by rule, regulation, law and/or statute to qualify for the role in their state.
- Certified Home Health and Oasis experience is strongly desired.
- Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care is a plus.
- Good assessment and clinical evaluation skills.
