Per Diem opportunity for an LMSW in the Richmond area!
Constellation Hospice is looking for a compassionate and empathetic Social Worker (LMSW) to work with our hospice patients. This role will involve providing care in the patient's homes and at Skilled Nursing Facilities.
Qualifications:
- LMSW license in the state of Virginia with at least 1 year of clinical experience.
- Hospice, Acute Care, and/or Bereavement Counseling experience.
- Have reliable transportation and be willing to commute within a designated territory to provide the best in-home patient care.
Responsibilities:
- Provide Clinical Social Work to our hospice population. Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, neglect or abuse and plans intervention based on evaluation findings
- Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan
- Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record-keeping system. Submits all clinical documentation, time records and other information in the required time frame
