Home Health Scheduler
Constellation Health Services is seeking a Scheduler for our Certified Home Health Program. The Scheduler is responsible for developing and managing efficient schedules for nursing staff, home health aides, therapists, and social workers based on patient needs, staff availability, and regulatory requirements.
Responsibilities:
Scheduling Coordination
- Develop and manage efficient schedules for nursing staff, home health aides, therapists, and social workers based on patient needs, staff availability, and regulatory requirements.
- Adjust schedules as needed to accommodate changes in patient care needs, staff availability, and unforeseen circumstances.
Data Management
- Maintain accurate records of staff availability, patient assignments, and scheduling changes using scheduling software.
- Analyze scheduling data to identify trends and areas for improvement in efficiency and service delivery.
Qualifications:
- High school diploma or equivalent. Associates degree is preferred.
- One or more years of working in an office setting. Previous scheduling experience in a healthcare setting is preferred. Home health scheduling is strongly preferred.
- Previous experience with EMR systems (e.g. Epic, HCHB, MatrixCare, Wellsky, etc.) preferred.
