Constellation is looking to add a School-Based Social Worker to our team who will work collaboratively with the school team and other therapists to provide services to children, including intervention in small group and individual sessions. Role Overview:
Aligned with the school calendar, the School Based Social Worker will provide services to school-aged children, conducting evaluations, implementing interventions, leading small group sessions and collaborating with school teams, families and therapists. Attendance and contribution to IEP meetings are integral to this role.
Responsibilities:
- Conduct comprehensive evaluations
- Screen students for referral to School Social Worker.
- Provide individual and group counseling services to students addressing a range of concerns, including anxiety, depression, ADHD, social-emotional difficulties, and behavioral challenges.
- Develop and implement supports for students and provide consultation to teachers and staff.
- Conduct crisis intervention and support for students and staff as needed.
- Collaborate effectively with teachers, parents, administrators, and other professionals (e.g., special education teachers, counselors, social workers) to create a supportive learning environment.
- Participate in Individualized Education Program (IEP) meetings and assist with the development and implementation of IEPs and 504 Plans.
- Build strong relationships with families and provide guidance and support to parents on child development and parenting strategies.
- Participate in school-wide initiatives and professional development opportunities.
- Complete all documentation in a timely basis including evaluations, progress reports, discharge summaries, statistical reports (all as required by the district).
- Provide in-service training to classroom teachers (as needed).
- Prepare materials to be used in direct intervention, classroom programs or home use.

Qualifications:
- Strong oral and written communication skills.
- Must possess or be eligible for Connecticut School Social Worker certification.
- Ability to relate successfully with staff, families and students from diverse backgrounds, experiences and skill levels.
