Position Overview:
The CTI Team Leader will be responsible for the operational and clinical management of the CTI component of the ESD program, ensuring that the CTI model is implemented with fidelity and that participants receive high-quality, person-centered support. The CTI Team Leader will oversee the care transitions of individuals leaving hospital or institutional settings, coordinating assessments, service linkages, and community integration efforts. This individual will supervise the Care Manager and other team members, ensuring alignment with recovery goals and timely delivery of services. The role will also involve quality monitoring, reporting, and collaboration with other agencies.
Essential Responsibilities:
- Oversee the day-to-day operations of the CTI component, managing care transitions and ensuring alignment with the phased CTI model.
- Lead intake assessments and coordinate the development of individualized care plans, tailored to the specific needs of each client.
- Ensure effective collaboration between hospitals, community providers, and CPU’s clinical teams, facilitating seamless transitions and service continuity.
- Supervise the Care Manager and other direct service staff, providing guidance and support to ensure high-quality service delivery.
- Monitor client outcomes, program fidelity, and adherence to performance metrics, making data-driven adjustments as needed.
Qualifications:
- Education: LCSW or LMHC required.
- Experience: Minimum of five (5) years in care management and coordination within behavioral health and I/DD fields.
- Skills: Expertise in care coordination, community integration, and recovery-focused interventions. Strong supervisory and communication skills.
Location: Multiple, including, but not limited to, CPU Hudson Valley Office, Yonkers Transitional Residence, and community-based locations, including hospital referral sites
Salary : $80,000
