Improve the health of communities and individuals by screening for Health-Related Social Needs (HRSN).
Constructive Partnerships Unlimited is hiring Social Care Navigators to improve the health of communities and individuals by screening for Health-Related Social Needs (HRSN), connecting individuals to appropriate services, and providing health education and community resource support. You will serve as a liaison between the healthcare system, social service providers, and community-based organizations to coordinate access to resources and improve service delivery quality and cultural competence.
Responsibilities:
- Work with Medicaid Members to connect them to the appropriate services to address their HRSNs that align with the Members’ preferences, limitations, disabilities, etc.
- Provide outreach, eligibility assessment, referral management, care coordination, and education. Confirm with the Member whether the referral was accessed and whether their needs were met. Interview participants to obtain basic data, and past medical history, and determine income status/program eligibility.
- Coordinate, if applicable, the Member’s benefits program application assistance and provide connection to clinical care management.
- Developing Social Care Plans for eligible Members that include a summary of Member needs, eligibility, and services to which Members are referred.
- Monitors and responds to communication(s) received from Members made via phone calls or from the Care Compass Collaborative website.
- Establish and maintain relationships with community agencies, service providers, and stakeholders to stay informed about available resources and referral pathways.
- Knowledgeable about community resources appropriate to the needs of participants/families and act as an advocate and liaison between the participant/family and community service agencies.
- Collect and analyze data on Member outcomes and service utilization to inform program evaluation and improvement strategies.
- May assume additional responsibilities, as required.
- MUST Maintain HIPAA compliance at all times.
MINIMUM REQUIREMENTS:
- Possession of a high school diploma or equivalent; AND two (2) years of work experience in the health care, community health, or social services field; OR two (2) years of college-level training in health occupations may substitute for one (1) year of work experience.
- Familiarity with the U.S. healthcare system and federally funded healthcare programs.
- Possesses strong communication skills (written and verbal)
- Negotiation skills and the ability to develop strong working relationships
- Proficient in planning & time management
- Ability to work independently with minimal supervision
- Utilizes critical thinking skills
- Possesses adaptability skills (both in attitude and action)
- Must have strong personal computer program skills including Microsoft Outlook, Microsoft Office (Word, Excel PowerPoint etc.)
- Facilitation of meetings either in person or via virtual platforms such as Zoom
