JOB SUMMARY
The Service Coordinator Supervisor monitors and oversees clients’ in-home care needs in compliance with Federal/State Regulations and program rules. Supervise program office staff and the daily intake and referral processes to ensure positive business growth and development. Responsible to behave in a manner that will promote cohesiveness to the office environment, as well as support day-to-day operations, and client advocacy.
JOB DUTIES
- Train Service Coordinators and other assigned employees in company Policies and Procedures
- Supervise and coach a team of assigned employees
- Conduct advanced reporting and oversight activities for daily operations
- Supervise and Coordinate the enrollment of clients and/or employees in services
- Oversee client assessments and home visits, as needed
- Communicate effectively with referral sources, clients and employees
- Conduct business in accordance with state, federal, program and contract requirements
- Maintain necessary skills and knowledge to coordinate work flow
- Exhibit computer efficiency
- Demonstrate dependability
- Demonstrate effective problem solving and decision-making skills
- Monitor deliverables and ensure timely completion of projects
- Participate in and assign professional development and training activities
- Collaborate with stakeholders regarding delivery of services
- Represent company at stakeholder meetings, health fairs, job fairs and provider fairs
- Market services to referral sources
- Handles complex service programs and special projects
- Other duties as assigned
QUALIFICATIONS
- Bachelor’s degree in the field of Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology or Related Field with one-year experience working with individual(s) with disabilities or the elderly preferred
- Meet requirements as outlined by program contracts
- One (1) year of management experience preferred
- Must maintain reliable transportation and valid insurance and driver's license for in-state travel
- Must be able to successfully pass a background screening
The incumbent typically works in an office environment and uses a computer, telephone and other office equipment as needed to perform duties. The noise level in the work environment is typical of that of an office. Incumbent may encounter frequent interruptions throughout the workday.
