Mental Health Care Coordinator:
We are seeking a skilled Clinician to join our team as a Mental Health Care Coordinator. In this role, you will be responsible for providing clinical services, including conducting assessments, developing treatment plans, and coordinating care with other providers.
Responsibilities:
- Developing and implementing individualized treatment plans to meet the unique needs of our clients
- Collaborating with multidisciplinary teams to coordinate care and ensure seamless transitions between services
- Providing on-call support and crisis intervention to address emerging issues
Qualifications:
To be considered for this role, you will need a Master's degree and CT State Licensure (LCSW, LMFT, LPC). Bilingual Spanish/English proficiency is highly desirable.
