Key Responsibilities:
Client Intake and Assessment:
- Conduct comprehensive intake interviews to evaluate clients' behavioral health needs.
- Gather and review medical, psychological, and social history to ensure accurate assessments.
Case Management:
- Develop individualized care plans tailored to clients' unique needs and goals.
- Coordinate services, including therapy, medical care, housing assistance, and vocational training.
- Monitor progress, reassess goals, and adjust care plans as necessary.
Resource Coordination:
- Connect clients with community resources, including financial aid, food assistance, and legal services.
- Advocate for clients in accessing mental health and social services.
- Collaborate with interdisciplinary teams to ensure comprehensive support.
Documentation and Compliance:
- Maintain accurate, up-to-date records in accordance with HIPAA and agency standards.
- Track and report data on client outcomes and service utilization.
- Ensure compliance with state and federal regulations related to behavioral health.
Support and Communication:
- Serve as the primary point of contact for new clients and their families.
- Facilitate smooth transitions into care programs, providing clarity and emotional support.
- Educate clients on their care options, rights, and responsibilities.
