What are the responsibilities and job description for the Intensive In-Home Team Lead position at Continuum Care Services, Inc. - Charlotte?
The IIH Team Leader determines , through comprehensive clinical assessment and face-to-face interaction/evaluation, services that best meets the therapeutic needs of the consumer. The team leader must be a licensed professional and is responsible for coordinating the initial assessment and developing the youth’s Person Centered Plan (PCP). The service model requires that intensive in-home staff provide 24 hour coverage, 7 days per week 365 days per year. The licensed professional is also responsible for 1) providing individual and family therapy for each youth or other family members served by the team. 2) Designating the appropriate team staff such that specialized clinical expertise is applied as clinically indicated for each child, 3) providing and coordinating the assessment and reassessment of the recipient’s clinical needs with the recipient, 4) providing clinical expertise and guidance to the IIH team members in the team’s intervention with the recipient, 5) and providing the clinical supervision of all members of the team for the provision of this service. An individualized supervision plan is required for all IIH team members exclusive of the Team Leader.
