What are the responsibilities and job description for the Community Support Team Para Professional position at Continuum Care Services, Inc. - Concord?
CST services are delivered to recipients, with a team approach, and within the limits of the approved policies and procedures of Continuum Care Services, Inc. and any outside regulating body. The Paraprofessional (PP) is responsible for assisting with the day-to-day operations of activities and programs for the benefit, support, and enhancement of life quality for individuals supported primarily in their living environments and include but are not limited to behavioral interventions such as modeling, behavior modification, and behavior rehearsal. CST is an intensive community-based rehabilitation team service that provides direct treatment and restorative interventions as well as case management. This team service includes a variety of interventions that are available 24/7/365, and are delivered by the CST staff, which maintain contact and intervene as one organizational unit.
Paraprofessionals are to model and contribute to the essence of home allowing opportunity for personal choice and preference for all clients. Knowledgeable and supportive of the mission, vision, philosophy, and polices and procedures of Continuum Care Services. Paraprofessionals support opportunities, activities, and programs instrumental in meeting goals according to the Person Centered Plan (PCP). The Paraprofessional endeavors at all times to promote the independence and individuality of each individual supported according to the PCP.
