Social Worker needed to support long term care facility in Fairbury, Nebraska. '..Identifies personal and medically related social service needs of the resident and assures that the individual needs are met by coordinating services with appropriate disciplines. Makes appointments and provides transportation to meet the medical and personal needs of the resident. Documents assessments, care plans and social service interventions per facility policy as appropriate. The Social Services Coordinator or Social Services Designee position also coordinates PIP teams, does QA audits, and attends and participates in weekly care plan meetings. At times, Social Services Coordinator or Social Services Designee may be pulled to work directly on the floor to ensure adequate care for the residents..'
