Continuum of Colorado has an opening to be our next Home and Community Support Supervisor . As our Home and Community Support (HCS) Supervisor, you’ll get to oversee the day-to-day of our Home and Community Support Providers in Aurora and Denver by directing your team’s duties and assuring quality care is provided to all of the individuals we serve by completing on-site visits to the homes of the providers / individuals. The HCS Supervisor will have flexible schedule in order to be available to staff and to monitor staff trainings, review documentation, review schedules, review and approve timecards, and supervise the staff’s adherence to the individual in service’s Service Plan / Plan of Care.

This is a hybrid remote position with plenty of flexibility!

Requirements

  • Two (2) years supervisory or management experience, preferably in developmental disabilities or in a Home Health Agency
  • Excellent oral and written communication skills; ability to effectively communicate at all levels with managers, subordinates, individuals in services and others
  • Work independently with minimal supervision
  • Work flexible hours, as needed
  • Meet the physical demands of the position including the ability to push and secure wheelchairs and assist in transfers
  • Valid Colorado driver's license and proof of auto insurance
  • Meet our driving requirements - no suspensions / revocations and no more than two (2) moving violations in the past three (3) years

Preferred

  • Bachelor’s degree or two years advanced study at an accredited college or university preferred
  • Working knowledge of the I / DD system, rules, and regulations

  • Additional Responsibilities

  • Ensures each staff follows agency policies and procedures that meet State regulatory guidelines
  • Assists Regional Manager in completing personnel related documentation (to include review and approval of timesheets); monitors staff attendance, punctuality, and use of available leave
  • Assures relevant day-to-day and emergency information is communicated to families, Continuum of Colorado Management, and other involved agencies and their respective staff
  • Identifies and resolves any concerns of the individual in services and / or families
  • Attends and participates in individual in services meetings
  • Develops and maintains individual in services and staff files
  • Prepares, reviews, and tracks documentation, including case notes, incident reports, critical incidents, and consumer satisfaction, as well as supervisory assessments, annual assessments, monthly service records, and Service Plans / Plan of Care
  • Works with Regional Manager to prepare and present Human Rights Committee reports
  • Prepares and / or assures documentation is complete in preparation for Service Plan Meetings as appropriate
  • Writes ISSPs and protocols for individual individuals in services per individual plan, and trains staff on how to implement ISSP; conducts annual follow-up for ISSSP
  • Follows agency policies and procedures regarding instances of mistreatment, abuse, neglect and exploitation (MANE) Critical Incidents and Occurrence Reporting
  • Salary : $21 - $24
