What are the responsibilities and job description for the Enteral - Patient Care Representative position at ContinuumRx?
The Enteral PCR will work under the supervision of the VP, Operations as well as with the Registered Dietitians.
- Call enteral patients monthly for supply refills.
- Conduct a nutrition screen during monthly refill calls.
- Consult DTR and RDs if issues (intolerance, weight change, constipation, diarrhea, nausea, vomiting, etc.) noted from nutrition screen.
- Document in the medical record the coordination of care with patients and other healthcare providers.
- Assist with obtaining lab results and attaching to medical record as needed.
- Responsible for obtaining DWO’s, requesting medical records, and obtaining new orders when authorizations are expiring.
- Enteral pump troubleshooting with patients and/or caregivers for Kangaroo and Infinity pumps.
- Provide excellent customer service.
- Other duties as assigned.
- Complete understanding of confidentiality with respect to company proprietary information as well as information concerning patient/client care; complying with all federal and state laws as apply to confidentiality of protected health information (PHI) and electronic protected health information (EPHI); and following the HIPPA guidelines regarding readily identifiable protected health information.
