Contra Costa Youth Service Bureau is excited to announce an opportunity for a Wraparound Care Coordinator within our organization.
About Contra Costa Youth Service Bureau: Contra Costa Youth Service Bureau is committed to promoting and providing family-centered services to targeted children and youth ages 6 to 21 that support the whole person and their families in Contra Costa County.
The agency was founded in 1984 by the West Contra Costa Consortium for Children and the Contra Costa County Probation Department. The Bureau was formally established as a community based non-profit agency in 1985.
The Program:
Contra Costa Youth Service Bureau offers "Wraparound" services to children and youth susceptible to placement, and to their families. The Wraparound programs are strengths-based, family-driven, flexible, and creative, with the goal of helping the family develop the skills and supports to prevent or reduce the possibility of out-of-home placement of their child. A family-based team addresses the needs of the entire family, not just the child of focus. This family-based team consists of family members, Contra Costa Youth Service Bureau's Therapists, Support Counselors, Family Partners, Facilitators, and other community members who have an important role in the family.
The Role:
This role will be responsible for working under the wraparound practice model of care, which includes a unique set of values that emphasizes individualized, strength-based, trauma-informed, community-based, and culturally intelligent care, provide intensive and individualized care coordination for children and adolescents.
The Wraparound Care Coordinator will deliver culturally responsive services and lead a collaborative planning process for youth and families who are referred to through Child Welfare, Juvenile Probation, or County Behavioral Health departments. Services will be provided exclusively in the community: in families' homes, foster homes, schools, other community locations, etc. for youth who could require being placed in out of home care or are stepping down from a higher level of care.
The primary role is to ensure a sustainable network of unpaid supports (i.e., extended family, mentors, teachers, coaches) are working together toward stability for the youth and family.
Key Qualifications:
- Working knowledge of positive child/youth development; patience and understanding of traumatic life situations.
- The ability to interact with escalating youth and/or parents in a calm and professional manner.
- Proven ability to respond appropriately in crisis situations.
- The ability to multi-task; sensitivity towards cultural, ethnic and disability issues.
- Proven ability to follow oral and written instructions.
