Key Responsibilities:
- Provide comprehensive case management services, coordinating patient care and facilitating discharge planning.
- Perform utilization review and manage the continuum of care across multidisciplinary teams.
- Effectively communicate with healthcare providers, patients, and families.
- Adaptable to multiple tasks and schedules, maintaining a positive and professional attitude.
- Minimum Experience: 2 years of experience in an acute care setting
- Required Skills: Ability to multi-task, work independently, strong communication, time management skills, and a teamwork approach
- Certifications Required: ACLS, BLS
- Scheduling Requirements: Monday - Friday, 8:00 AM - 4:30 PM shifts, every other weekend
- Submission Requirements: ID required for address verification, Meditech experience
- Flu Vaccine Required Year-Round
- Must Reside Outside a 50-Mile Radius of the Facility
- Estimated Hourly Rate: $49
- Weekly Stipend: $1,215.80
- Gross Weekly Pay: $2,016
- Recruitment Bonus: $1,260 (for a 13-week contract, prorated for shorter durations)
