Position Summary:
The role of the CMC Home Hospice Liaison is to promote the mission, services, and products of CMC Home Hospice and all other services and benefits of referral sources and the community. Positive public relations and reputation in the community are key responsibilities of the HL. The HL is to grow their census and be a positive reflection of CMC while being heavily involved in the community.
Job Duties:
- Provide constant face to face contact with community referral sources both healthcare and non-healthcare related.
- Examples of healthcare-related avenues to be marketed include but are not limited to hospitals, discharge planners, nursing and assisted living facilities, physician offices, pharmacies, sitter service centers, all other facilities/agencies, etc.
- Examples of non-healthcare-related avenues to be marketed include but are not limited to senior centers, community agencies, veteran agencies, libraries, shopping centers, etc.
- Develop strong relationships with new referral sources and closely monitor and foster already-existing relationships with previous referral sources.
- Introduce and promote primary relationships of potential referral sources.
- Oversee and monitor primary relationships for quality and consistency.
- Submit in a timely manner written documentation of all contacts, referrals, and contracts. Information will be submitted in CRM within a (24) twenty-four-hour timeframe.
- Develop activities and events that will lead to an increase in census and positive public relations.
- Make appointments with referral sources and follow-up on them timely and effectively.
- Make frequent regularly scheduled visits and contacts with appropriate referral sources.
- Implement and maintain brochures and educational material in appropriate places.
- Provide Home Hospice return on investment and be a quality steward of company funds.
- Assist in orientation of new employees.
- Market and provide community education.
- Participation and marketing in public relations and community activities that promote the organization and its role as an effective member of the hospice and healthcare delivery system as well as customer service promotion.
- Practice effective communication to both external and internal customers.
- Communicate appropriately and clearly to patients, family members, staff, physicians, etc.
- Demonstrate an ability to be flexible, organized and function under stressful situations.
- Maintain a good working relationship both within the department and with other departments
- Represent the organization in a positive and professional manner.
- Must maintain high ethical standards.
- Must exhibit ability to function with minimal supervision
Qualifications:
Education:
- High School Diploma or equivalent required.
- Bachelor’s Degree in Marketing or Healthcare Promotions preferred.
Experience:
- Previous clerical/administrative experience in an office setting required.
- Minimum of three (3) years’ experience in a Hospice/Healthcare setting preferred.
- Demonstrated ability to supervise and direct professional and administrative personnel.
- Knowledge of business management, governmental regulations, and Medicare Conditions of Participation (COP) standards.
- Ability to read and communicate effectively.
- Strong written and verbal skills.
- Basic computer knowledge.
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
- Ability to foster a cooperative work environment.
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment.
- Must be able to work flexible hours and travel between offices, facilities, etc.
- Must be a licensed driver with an automobile that is insured.
- Understand hospice philosophy and issues of death and dying.
