Job description
Position: Travel- Registered Nurse Care Manager -Hospice
Shift: Mon - Fri, 8 am - 5 pm, 40 hours per week
Duration: 13 Weeks, with extension possibility
Scheduled Hours: 40 Hours
Location: Fort Wayne, IN
Pay Breakdown:
- Pay Package (36 hours):
- Total Non-taxable Pay (weekly): $1360/-
- Total Taxable Pay (weekly): $1240/-
- Gross Total Pay (weekly): $2600/-
Responsibilities
- Conduct comprehensive patient assessments to develop and implement individualized care plans.
- Coordinate with healthcare teams to ensure effective managed care and discharge planning.
- Provide education and support to patients and families regarding treatment options and care processes.
- Maintain compliance with HIPAA regulations and medical terminology standards.
- Utilize knowledge of physiology and hospital experience to enhance patient service and outcomes.

Requirements:
- 2 Years of recent Experience - Required
- Ready to start ASAP
Certifications:
- BLS
- IN RN state license
