North Shore Health is seeking applicants for the Social Services Coordinator position. The Social Services Coordinator is responsible for all social service activities within the hospital, care center and home care; manages the operations of the social services department; provides social service assessments, coordination, and referral services to patients and residents; participates in providing staff education and support; participates in interdisciplinary teams and other committees as assigned. The facility social services coordinator must maintain a high level of professional knowledge and expertise in relevant program areas.
Minimum Qualifications (Preferred Qualifications)
Experience: Three (3) years in Human Services (Five (5) years of Medical Social Work)
Education: Bachelor’s Degree in a Human Services field including, but not limited
to, sociology, gerontology, psychology, etc.(Bachelor’s Degree or higher in Social Work)
Licensure/Certification/Registration: N/A (MN Social Work License)
