With a full future earning potential of $65,780.40 -$90,043.20 annually. This position is a full-time 40 hours per week and eligible for full-time county BENEFITS PACKAGE. Job Summary : Performs work in providing social work services for families, children and individuals experiencing complex social, mental, physical and emotional health situations and requiring the development of and the need for high level of support to actively engage complex social treatment plans directed specifically at the prevention of child maltreatment; performs and is responsible for assessment and casework, directs service programs; and related work as assigned. Essential Functions / Responsibilities : The following duties are normal for this classification. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.1. Family Services Case Managementa. Provides social work case management services for children and families experiencing complex social, mental, physical and emotional health circumstances.b. Assesses, develops, implements, and evaluates multi-faceted treatment plans.c. Researches, identifies alternatives, and develops appropriate resources and service plans within departmental policies and MN state statutes in consultation with supervisory staff.d. Provides counseling, coaching, and psycho-social education to support emotional regulation of children and families that promote stability and safety.e. Travels in and outside of county to conduct regular home visits; initiates and participates in community-based activities; conducts on-the-job visits, school visits, and medical and behavioral health appointments as appropriate. f. Examines and monitors medical, educational, and therapeutic goal attainment while a child is in out-of-home placement. g. Monitors and conducts ongoing assessment of service and treatment delivery, appropriateness, and effectiveness over time using consultation, and in some situations uses persuasion, to achieve desired outcomes. h. Evaluates, participates in, and develops processes for coordinated and integrated services between children and adult services, home and community-based services and probation staff to support the overall stabilization of the family unit.i. Communicates and collaborates with the county attorney's office to address child safety and permanency needs through court actions. Advocates and promotes and persuades, as necessary, the use of effective court ordered services for families and children. Prepares and provides testimony, research, and service recommendations. Provides advice and updates on rule changes and treatment requirements related to court ordered services. Collaborates with all supervisory staff to assure proper referrals for on-going case management needs. 2. Assessment and Investigationa. Makes child safety paramount and at the forefront of decision making.b. Assesses and ensure the safety of a child initially and ongoing during involvement following the rules and laws of the State of MN, utilizing decision making tools and practices and in consultation with team / supervisor. Through direct observation, interviews, and document review, gather facts to help determine if a child has experienced harm, assess / evaluate risk for subsequent child maltreatment.c. Through direct observation, interviews, and document review, gathers facts and assess family protective capacities related to child safety.3. Intakea. Assesses and actively responds to a diverse array of phone and face to face intake calls for children referred for child welfare, children's mental health services, child protection, adult services, developmental disability services, as well as inquiries about housing, employment, and / or financial assistance. b. Utilizes Intake methods including but not limited to person-centered interviews, home visits, and collateral / provider contacts and records review.c. Provides on call response to child protection reports as required by the MN DHS Maltreatment of Minors Act.d. Provides early intervention and assistance with people needing services outside of the regular working hours as assigned. e. Assesses for personal and physical safety of callers and community and creates safety plans accordingly. f. Initiates communication and coordinates services with local, regional, state resources including Law Enforcement, County Attorney and Probation. 4. Documentationa. Records all client contact, collateral, and consultation via the Social Services Information System (SSIS). b. Maintains and updates client contact information, medical and mental health records in SSIS.c. Prepares, completes, and maintains mental health and child protection crisis plans, and screening documents. d. Documents all correspondence and communication in accordance with county, state, and federal guidelines. e. Responsible to document in accordance with HIPAA and data privacy compliance standards.f. Organize and maintain back up paper files on medical, mental health, school, law enforcement and court records as needed.g. Completes and maintains specific Indian Child Welfare Act (ICWA)-related documentation in accordance with laws and rules of State of MNh. Prepares, completes, maintains, and submits court reports as requested.5. Case Reviews Team Consultation. Collaborates with team members, outside agency consultants / providers and supervisory staff to review and analyze service delivery utilizing a case review methodology to make service recommendations.6. Crisis Response and Interventiona. Provides crisis intervention in-person or over the phone. Responds quickly to assess lethality, takes action steps (e.g., accompanying to ER, determine placement) and creates safety plans to restore regulation of client.b. Identifies after-hours crisis resources for families.Non-Essential Functions : 1. Emergency Response - Trains, practices, and responds to all hazard events for mass care and mass dispensing.2. Attends trainings / seminars to maintain skills and competence and attends all-staff meetings to engage and contribute to PHHS related activities. 3. Perform other related duties as assigned.Minimum Qualifications : Education and Experience

  • Four (4) year college degree from an accredited college or university with a major in social work, psychology, sociology or closely related field; and
  • Two (2) years of relevant experience.
  • Or an equivalent combination of education and experience sufficient to perform the essential functions of the job. Must pass the Merit System competency test in order to be qualified for the job.

  • SUD / LADC Service ProvidersWorking Environment :
  • Work is performed primarily in clients' homes, agency, schools, community mental health agencies, treatment facilities, and court.
  • Frequent travel to meetings, trainings, and client homes up to 500 miles; may involve exposure to inclement weather conditions, and regularly involves working in homes that are unsanitary and unsafe environments including exposure to secondhand and thirdhand smoke, bedbugs, lice, garbage and / or bodily fluids.
  • Direct service provision requires dealing with persons who may be hostile, aggressive, and abusive or pose threatening conditions including physical, mental, and emotional health hazards.
  • Exposure and experiences of secondary trauma.Physical Requirements Of This Position : Sit or stand at a desk for extended periods of time and perform work at a computer or talk on the phone.Hear and speak effectively to communicate in person and over the phone.Visual acuity to visually evaluate clients, read computer screens and printed documents and to see to drive a motor vehicle.Apply physical coordination in order to safely drive an automobile throughout the county under various road and weather conditions. Physical ability to move about the office and to traverse the various work sites.Manual dexterity to type on a keyboard and calculator and use a computer mouse; manual tasks such as handwriting, searching through documents and compiling material.
  • Office work may include reaching for documents and supplies, standing or kneeling to search through file cabinets, using fingers to search for files, grasping documents, and repetitive motions such as typing and sorting / compiling material.
