LTC / SNF LPN position at Cook Hospital
Type of Facility : 1 unit; short term rehab, LTC, SNF, no locked memory. 28 beds, private rooms.
Common diagnosis : Dementia, COPD, CHF, DM, HTN, and more.
Job Duties : LPN : follow up assessments, med pass, charge oversees CNAs, Skin assessments, Pain assessments, admissions, wound care, pressure ulcer prevention / treatments, assisting other staff as needed; family involvement and communication.
Job Requirements
Required Certifications : BLS; No IV Certification needed for LPNs.
Experience : Accepts first time travels.
Schedule Information
Day shift : 0600-1430
Evening shift : 1415-22
Night shift : 2230-0700
Can mandate to stay 4 hours past shift for overtime per union rules.
May be asked to rotate between different shifts to meet needs.
Weekend Requirements : EOW
Holiday Requirements : May be asked to go into the Holiday Rotation.
Unit Specific Information
Interdisciplinary Team : PT, OT; Daily Meeting.
Manager : Theresa (available as call); Charge- RN or LPN.
Physician availability after hours : 24 / 7 on the hospital side.
Additional Details
BLS; 2 / 10 START DATE
EMR : Meditech will be switching to a new system in June 2024.
Ratios & Staffing : LPN : 8 hour shifts; covering the other 16 hours of the day; Ratio 1 : 23-28.
Lifts : mechanical total lift system.
Pharmacy on-site? : Bubble Packs, Pharmacy onsite on hospital side.
Department Specific Orientation : 1 / 2 Day General, 1-2 days with Educator, 1-3 with a preceptor on designated shift.
