GENERAL SUMMARY
The Community Health Coach will provide lay coaching to Jefferson County residents 18 years of age or older related to health care and chronic disease management. This role works jointly as part of the Cooper Green Clinical Support Operations team and with the Live Health Smart Alabama team.
As part of the Clinical Support Operations team, the Community Health Coach will assist and educate patients by facilitating referrals, explaining medication regimens, facilitating with enrollment in applicable classes or appointments with Cooper Green partners, such as the patient educators, dieticians, social workers, pharmacy, or other services and screenings as needed.
Additionally, The Community Health Coach will work within seven (7) Jefferson County zip codes, connecting the residence in those areas to a primary source of care. This role will educate prospective patients on enrollment at Cooper Green and the importance of having a primary care provider, preventative screenings, and immunizations.
KEY RESPONSIBILITIES:
1. Effectively communication with patients and their caregivers, providing instruction, coaching, and
suggestions in partnership both internal partners and Live Health Smart Alabama.
2. Assists with quality improvement and research projects as needed; identifies obstacles to improvement. Identifies and communicates opportunities for process improvement.
3. Completes and submits reports, based on work being done in the community.
4. Encourages and facilitates enrollment to Cooper Green.
5. Utilize motivational interviewing skill set to solicit health and determinants of need of those coach is in
contact with.
6. Networking relationships and partnerships with key organizations, programs, and people in the targeted communities, leveraging opportunities to connect with residents.
7. Record the names and contact information of community members who would benefit from Cooper
Green enrollment. Proactively assist with enrollment when appropriate.
8. Performs other duties as assigned.
Position Requirements:
MINIMUM QUALIFICATIONS
Position requires high school diploma or GED
Has an unrestricted driver’s license (please note that a Motor Vehicle Records History will be part of the
background check requirement).
Must have adequate transportation that is covered by auto insurance
Must possess basic knowledge of computers, PC applications, and software
PREFERRED QUALIFICATIONS
Experience in a medical setting or in a care-giver role
WORK ENVIRONMENT
This role may require lifting up to 50 lbs.
The work location for this role is a hybrid of onsite work and travelling within Jefferson County.
