GENERAL SUMMARY
The Patient Lay Navigator provides information, support, and assistance in overcoming barriers to achieve the most optimal level of healthcare. This position may assist various organizational programs such as specialty clinics, behavioral health, primary care, health equity, patient outreach and more.
Primary responsibilities include locating resources and services, serve as a liaison, advocate, and facilitator. The Lay Navigator will encourage patient engagement in care, monitor compliance, and encourage adherence to treatment plans. The Lay Navigator can deliver services both directly and indirectly to members, participants, and patients in collaboration with clinical and non-clinical areas, as well as internal and external healthcare entities, and community organizations under the guidance of the nurse supervisor and/or provider in accordance with facility policies and procedures.
The lay navigator demonstrates a positive, dynamic, and empathetic demeanor supporting patients verbally over the phone and face to face. This position must demonstrate the ability to seek out additional support when needed, problem solve in an empathetic manner, and a willingness to service and engage with a diverse group of members/participants/patients. Excellent communication and customer service skills are paramount to successfully achieving positive patient outcomes.

KEY RESPONSIBILITIES
- Serves as an advocate for patients; provides advice, support, and direction, as well as to help patients identify resources that may be helpful to their situation.
- Understands the typical obstacles in navigating the healthcare system and works to help remove barriers to effective care by connecting patients and their families to appropriate resources.
- Asks questions and encourages patients to remain on their medications, get proper nutrition, engage in regular exercise, and continue with age-appropriate health screenings.
- Works with patients to help them access appropriate medical follow up, treatment and care.
- Performs effective communication skills; instructs patient/family per protocol or as instructed by professional nurse or provider; adheres to proper channels of communication in a courteous and cooperative manner; protects patients’ rights and confidentiality; handles telephone calls appropriately.
- Communicate with Care Team members (Care Coordinators, primary care physicians and other health care providers) to facilitate client care.
- Maintains accountability for a safe environment for patients and others. Adheres to safety regulations.
- Maintain accurate client records by documenting all engagement, services provided, and outcomes.
- Identify patients that could benefit from Lay Navigator services, complete initial needs assessment, and maintain follow-up to ensure appropriate guidance to healthcare services.
- Collaborate with Care Team (including Team Based Care, but not limited to, RN Coordinator and primary care provider) to discuss patient care issues and facilitate patient health care necessities.
- Ensure accurate documentation of all patient interactions and complete reporting requirements in accordance with organizational standards.
- Monitor patient attendance at medical appointments and navigation sessions, and initiate outreach and procedures for missed appointments as needed.
- Participate in meetings and in-service trainings at the request of or with the approval of a supervisor.
- Other duties as assigned.
Position Requirements:
MINIMUM QUALIFICATIONS
WORK ENVIRONMENT
This role may require lifting up to 50 lbs.

MINIMUM QUALIFICATIONS
- Associate’s degree in a healthcare related field, and at least six months of experience.
- Current CPR/BLS certification via the American Heart Association or American Red Cross.
