Position Description:
GENERAL SUMMARY
Responsible for theoretical & technical knowledge in various fields of patient care, as well as consultant (evaluates patient’s needs) educate and research. Identifies and recommends clinical standards of practice for patient populations.
KEY RESPONSIBILITIES
1. Provide health and wellness education in helping patients and their families understand diagnoses and treatment.
2. Evaluate, design, present, recommend, and disseminate appropriate health education information and materials.
3. Provide patients with in-office education and/or via electronic means.
4. Organize campaigns to provide outreach education to patients and general public about health-related topics.
5. Assist with navigation of newly enrolled patients.
6. Facilitate classes based on identified needs.
7. Acts as a liaison between patients, providers, and nursing staff.
8. Other duties as assigned.
Position Requirements:
MINIMUM QUALIFICATIONS
Position requires Bachelor’s degree in Nursing and three (3) years’ experience. Work experience may NOT substitute for minimum requirement. A current Alabama RN license is required. BLS Certification required. Current CPR certification is required or must be obtained. Experience providing care to patients with varying disease processes.
Preferred qualifications
Prior experience as a patient educator/navigator; Health Education Specialist Certification; Minimum of five (5) years of nursing experience.
WORK ENVIRONMENT
This role may require lifting up to 50 lbs.
The work location for this role is onsite.
