Job Description
The Case Manager Registered Nurse will coordinate discharge planning to reduce LOS and improve throughput, adding financial stability to the organization. The nurse will work with the interdisciplinary Patient Care Team and Primary Care Physician to develop an in-hospital and post-discharge plan of care for patients.
Required Skills and Qualifications
To be successful in this role, candidates must have a Baccalaureate degree in Nursing from an accredited school or university, or equivalent RN degree with commensurate clinical experience. A minimum of 2-3 years of experience in a hospital setting is required, as well as a minimum of one year of experience in Case Management/Utilization Management or Home Care. Full-time employment with weekend and holiday commitments is necessary.

- Conduct admission and continued stay review
- Monitor utilization management and monitoring process
- Identify trends and patterns of resource use and patient care
