This position plays a critical role in ensuring continuity of care for patients with post-hospital health care needs. The Case Manager Registered Nurse will work closely with the interdisciplinary Patient Care Team and Primary Care Physician to develop an in-hospital and post-discharge plan of care for patients. This individual must have excellent communication skills and be able to navigate complex systems to ensure seamless transitions of care.
