About the Job:
This is a unique opportunity to join the Cooperman Barnabas Medical Center team as a Discharge Planning Nurse Specialist. In this role, you will be responsible for coordinating the care and services of patients across the continuum of illness, ensuring continuity of care for those with post-hospital health care needs.
Responsibilities:
- Collaborate with the interdisciplinary Patient Care Team and Primary Care Physician to develop an effective in-hospital and post-discharge plan of care.
- Coordinate patient care services, in conjunction with medical, nursing, and other ancillary staff members.
- Identify trends and patterns of resource use and patient care, making recommendations to optimize care delivery.
Requirements:
- Baccalaureate degree in Nursing from an accredited school or university, or equivalent RN degree with commensurate clinical experience.
- Minimum of 2-3 years experience in a hospital setting.
- Successful completion of all Orientation Programs.
- Certifications and Licenses Required: Current Registered Nurse License in the state of New Jersey, Basic Life Support (BLS) from the American Heart Association.
