Job Summary:
The Incident Investigator position investigates incidents consistent with CCANY’s Incident Management program and policies. Completes assigned projects and initiatives for the department. Actively utilizes systems including Incident Reporting Management Application (IRMA), CHOICES, and internal monitoring systems while adhering to the regulatory guidelines.
Essential Duties and Responsibilities:

Understands and applies governmental laws and regulatory rules; Keeps current on trends.

Applies sound investigative techniques and practices, investigates reported incidents assigned, complies with regulations, laws, and CCANY program policies, and procedures:

Makes recommendations for resolution of issues,
-
Tracks that corrective action/follow up have been completed, timely and as prescribed,
-
Maintains all documents, files, and records ensuring they are complete and secure,
-
Utilizes required databases to enter information timely, accurately, completely.

Protects the rights of individual’s served and makes sure informed consent measures are adhered to.

Participates on committees, in meetings, and presentations as assigned.

Performs other duties, as assigned.

Must possess a valid Driver’s License from New York, or a contiguous state (i.e., Connecticut, New Jersey, Pennsylvania, and Vermont) OR Must have the ability to take ample public transportation to attend meetings in person in the community and in the office as needed.
Qualifications
Education and Experience:

Bachelor’s degree, preferably in a Human Services, Health Care Management, or related field, and three (3) years experience in OPWDD, DOH, Incident Management, Compliance, and/or policies and investigations.

-Or- Master’s degree one (1) year as above

-Or- Associate’s Degree four (4) years as above
-
-Or- HS Diploma/GED five (5) years as above
-
Certified training in investigative work preferred but not required.

Excellent working knowledge of Microsoft Word, Excel.

Fluency or ability to rapidly learn applicable laws, regulations, policies, and procedures, to ensure proper and thorough inquiries.

Ability to communicate directly and professionally verbally and in writing, including and not limited to the completion of investigative or other reports.

Travel per business necessity to CCANY locations or elsewhere as directed, to execute duties.

Maintain comprehensive understanding at all times of the potential ramifications of potential errors, whether in the items being reviewed or the review itself, regarding the potential harm, both individually and organizationally.

Strong organizational and project management skills and demonstrated attention to detail.

Ability to use discretion and maintain the confidentiality of information obtained in conduct of work.

Excellent interpersonal, public speaking, and written communication skills.
-
Strong documentation, analytical, and problem-solving skills is required.

Experience working with Electronic Health Records.

Ability to work autonomously.
