Coordinated Care Services Inc (CCSI) seeks a talented Court Navigator to contribute their expertise in providing essential business services to our provider organizations and customers. CCSI is contracted by Onondaga County to provide program management services, including specialized staffing. The Court Navigator will be located in Onondaga County and is responsible for supporting justice-involved individuals with mental health needs who are currently in the community. Court Navigators will be housed within the local county government but work in close collaboration with the local courts within Onondaga County.
Court Navigators will be responsible for providing support and connections to community behavioral health resources, that support health wellness and recovery for those identified as having mental health treatment needs or a diagnosed serious mental illness.
The Court Navigator should possess a strong understanding of local mental health, social services, and criminal justice systems. Additionally, insight into the CPL 730 process and additional mental hygiene laws in relation to court proceedings will be important for the Court Navigator to possess. Court Navigators are responsible for the tracking, documenting, and reporting of screenings, interventions, and outcomes throughout their involvement with individuals.
In this role, you will:
- Conduct initial screenings to assist with referrals to mental health and/or substance use treatment services and to determine if a more comprehensive evaluation is needed.
- Connect individuals with appropriate mental health services, treatment programs, and community resources based on their needs and circumstances.
- Follow-up with referred individuals and community providers to ensure seamless transition and engagement in services.
- Develop a comprehensive resource guide of the local behavioral health and social services network to assist with matching individuals with appropriate resources to promote health, wellness, and recovery.
- Facilitate communication and collaboration with community behavioral health providers, criminal justice stakeholders and state agencies on mental health service-related issues.
- Coordinate with OMH Diversion Center to discuss system issues and develop innovative solutions to barriers; collect and submit data to OMH on navigator program referrals and outcomes.
- Coordinates the development of system-wide treatment and support plans that could include: treatment services, case management, housing, care management, other identified supports, as needed.
- Communicates effectively and professionally with the client, providers, families and criminal justice personnel to assist with developing a plan.
- Recommend adjustments to treatment plans, and advocates for system movement
- Works closely with local County, City, and Justice Courts throughout Onondaga County.
- Cultivates partnerships with community providers to best support clients.
- Participates in creation of community education materials and presentations.
- Collaborates closely with AOT, Forensic Transitions, and 730 team to ensure appropriate service delivery.
- Culturally responsive to the population being served in the development, design, monitoring, evaluation and/or implementation of programs and services.
- Attend staff and other community meetings.
- Supports clients in achieving their program goals.
- Travels as necessary to fulfill responsibilities.
- Identify creative solutions to complex problems utilizing available community resources
- Provides support to other programs as needed
- Flexible schedule to meet the needs of the program
- Delegated task as needed by Manager of Forensics
- Other duties as assigned.
Requirements
EDUCATION AND EXPERIENCE:
- Bachelor’s degree in human services field and 4 years of work experience or equivalent education and work experience (see CCSI's equivalency chart)
- Additional professional experience with the justice system, criminal procedure law, and/or mental hygiene law are preferred.
