PART-TIME [Nonexempt] : 20 hours per week

 

Coordinated Care Services Inc (CCSI) seeks a talented Violence Prevention Caseworker to contribute their expertise in providing essential business services to our provider organizations and customers. Rise up Rochester is a nonprofit that empowers the community to establish and maintain a nonviolent culture. Rise Up Rochester provides assistance to individuals affected by crimes and their families. The role of the Violence Prevention Caseworker at Rise Up Rochester involves dedicated support for our clients. As a caseworker, you will focus on delivering comprehensive case management services aligned with the objectives outlined in the safety plan. Your responsibilities will include ensuring the fulfillment of clients' fundamental needs and maintaining regular communication to monitor the implementation of their safety plan, while also aiding them in progressing towards personal sustainability.


 CCSI’s is contracted by Rise up Rochester Incorporated (RUR) to provide capacity building services, including specialized staffing. This position is employed by CCSI and working on behalf of Rise Up Rochester


In this role, you will

  

1. Casework Management:

  • Conduct thorough assessments of individuals affected by violence to identify their unique needs and challenges.
  • Develop and implement personalized case plans to address immediate safety concerns and long-term well-being.
  • Document consistently and accurately in records all contacts with clients.

2. Housing Solutions:

  • Collaborate with community resources and agencies to secure safe and suitable housing options for clients.
  • Advocate for clients in securing temporary and permanent housing solutions, addressing homelessness or unsafe living conditions.
  • Conduct safety checks when temporary housing is secured.

3. Hotel Safety Checks:

  • Coordinate with outreach workers to conduct safety checks in hotels to ensure clients are not damaging the property.
  • Work closely with hotel management to address any concerns and maintain positive relationships.

4. Basic Needs and Crisis Support:

  • Develop and implement a comprehensive plan to ensure that the basic needs of all clients are met.
  • Provide 20-30 hours per week in direct support related to crisis and response.

5. Guidance and Support:

  • Provide guidance to tenants in problem-solving, conflict resolution, and assistance with housing needs.
  • Offer case management to victims, their families, and neighborhoods directly impacted by a violent incident.

6. Privacy and Confidentiality:

  • Maintain the privacy and security of information provided by participants.
  • Ensure complete confidentiality of participant encounters, referrals, and case management records.

7. Community Collaboration:

  • Collaborate with other organizations engaged in community outreach to enhance collective efforts.
  • Participate in community events and initiatives aimed at violence prevention and support.

8. Other Duties:

  • Perform other duties as assigned, contributing to the overall effectiveness of the violence prevention program.

Requirements

Education and Experience

  • Minimum of Associate degree plus 5 years of work experience in crisis intervention and/or violence prevention  (see CCSI's equivalency chart)   


Additional Requirements

  • Satisfactory completion of all required background screenings.
  • Respect the confidential nature of all CCSI and CCSI customer information you are exposed to in the course of your work performance. Staff working directly with protected health information /electronically stored protected health information, personally identifiable information, and other patient/consumer/customer data, will abide by the specific procedures and policies outlined by CCSI’s Code of Conduct and CCSI’s Privacy and Security Policies, New York State regulations and Federal regulations.
