POSITION SUMMARY
The Family Support Specialist is responsible for providing high quality and comprehensive services to children and families enrolled in the Healthy Families Program through regular home visiting services.
CORE ACCOUNTABILITIES
Home Visit Goals
- Home visits should be family centered, strength based and directed at establishing a trusting relationship.
- Visits will primarily occur within the family's home and each visit should last at least one hour.
- Assist parents in improving their skills to optimize the home environment.
- Improve the family support system.
- Increase the family's ability to problem solve and assume role of advocate for themselves and their children.
- Identify needs and refer for other services as needed.
- Assist family in establishing goals and a plan for accomplishment of the goals.
- Assess the normal growth and development of target child.
- Maintains professional boundaries and families.
Recordkeeping and Following Guidelines
- Provide potential client with program explanation and benefits and enroll the family into program services.
- Establish rapport and a trusting relationship with families by providing regular/long-term contact according to guidelines and level of service.
- Assess all families within the target population according to the guidelines established.
- Complete assessments and screenings in the timeframe established in the policies and procedures.
- Enter all required information in the computer system.
- Maintain all paper and electronic files and ensure all information is up to date and correct.
Training and Professional Development
- Compete all trainings required by Healthy Families.
- Ensure that yearly trainings are completed by due date and record of training is provided to Program Supervisor.
- Participate in staff meetings, conferences, training sessions and workshops as assigned.
- Actively participate in regular reflective supervision.
Additional Duties and Responsibilities
- Demonstrate familiarity with policies, performance standards and the agency's Mission, Vision and Core Values.
- Be familiar with the HFI Program Policy and Procedure manual.
- Exhibit a positive, professional attitude and maintain friendly and respectful relations with staff, children and families.
- Be a contributory team member in a positive/productive manner.
- Maintain HIPAA and confidentiality.
- Perform other duties as assigned.
QUALIFICATIONS
- Associate's degree in Human Services, Child Development, Psychology, Education or related field or two years of related experience providing services to children and families preferred. Bachelor's degree in above mentioned fields is also preferred.
- Experience providing services to children and families home-visiting experience preferred.
- Understand the social and economic challenges of the service area and be familiar with the community resources available for families.
- Strong knowledge and commitment to serving families of diverse social and economic backgrounds.
- Individual must be accepting, nonjudgmental, supportive, and compassionate.
- Knowledge on infant and child development preferred.
- Experience in working with families with multiple needs preferred.
- Must be able to work independently while adhering to program guidelines and recordkeeping requirements.
- Valid driver's license and access to an insured vehicle required.
- Positive approach and ability to communicate effectively with families.
- Possess excellent, highly developed listening skills with reflective capacity.
- Proficient in both written and oral communication skills.
- Must be detail-oriented, able to think critically and efficiently organized with the ability to maintain detailed records.
