- Full-time Case Manager is needed for our Elkins, WV office.
- A Bachelor’s degree in the Human Service Field is required
- Work is with children and adults within the WV IDD Waiver Program
- Participate in Annual Assessments
- Develop individual service plans
- Facilitate IDT meetings
- Linkage and Referral
- Advocacy
- Crisis Response Planning
- Documentation of case management activities
- Travel to client homes for monthly visits, meetings and assessments
