Who We Need
Immediately Hiring a Community Health Worker for our Metro Pheonix location.
The Community Health Worker (CHW) serves the community as a bridge between the members and the health care and social systems (providers, community resources) through outreach, education, and resource connection. This position is primarily responsible for providing supportive care and assistance to a diverse member population identifying barriers to self-care, decision-making, and navigating the social and healthcare systems. The CHW assists with the company's HEDIS measurement goals by providing personalized support to help members achieve good health practices. The CHW conducts community outreach, home and hospital visits, collaborates closely with the identified care team, and consistently demonstrates a high standard of ethical behavior, professionalism, compassion, and confidentiality.

JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO)
Provide one-on-one non-clinical interventions, serving as an advocate for members to obtain needed health services and community resources.
