Job Title: Social Adult Day Care PROGRAM COORDINATOR
Reports To: Executive Director
Supervises: Social AdultDay Care Program Assistant (1.5 FTEs)
Job Summary:
To direct, lead, coordinate and provide services to SADC participants daily. Ensure all team members adhere to all regulations as they relate to social adult day care. Help to enhance quality of life of participating seniors, by planning and providing activities of daily living, stimulating engagement in activities and programs. Ensure all paperwork compliance inclusive of individual person center service plan for each participant. Network and partner with day program referral sources.
Duties and Responsibilities:
· Lead the day-to-day operation of the ASD program team inclusive of supervision, evaluation of Senior Day Program staff and volunteers. Plan programs and schedule activities to ensure maximum benefits to program participants while ensuring compliance with all related regulations.
· Planning and ensuring execution of all services related Participant Care Plans and the daily management of the Senior Day Program to include, but not limited to, intergenerational programs, day trips, special events, meals, snacks, transportation, arts, and crafts. Develop activities that support goals of individualized care plans.
· Ensure each participant has an individualized care plan to meet their cognitive, behavioral and physical skill level. Conduct annual care plan meetings with participants, staff, caregivers, and service coordinators using electronic record system
· Marketing of Senior Day Program to include speaking engagements, presentation at health fairs and conferences to ensure community understanding of social adult day care model. Continued improvement and updating of program brochure/material.
· Ongoing network with Medicaid/Medicare insurance companies, the office for aging, other agencies that serve older adults, etc. for additional funding sources for prospective participants/old adults needing support.
· Ensure the ongoing operation of MyAdultDayCare (electronic tracking system) to manage participant record keeping and data tracking ensuring regulation compliance; assess and reevaluate each participant individualized service/care plan, case notes, progress summary, including discharge planning.
· Develop goals and plans of service for each participant meeting each persons and/or care providers desired outcomes.
· Maintaining data to ensure proper billing and data tracking regarding service delivery, attendance days, bus rides and meals for each participant.
· Maintain contact with contracted service providers for case management of each participant to ensure continuity of care provided to include meals, transportation, and general well-being of the participant.
· Monitor physical and behavioral changes in participant with communications to caregiver regarding changes and concerns. Develop and maintain open communication with caregiver to ensure their needs and the needs of their loved ones are being met.
· Maintain open communication with Executive Director regarding daily operation and services of the Senior Day Program.
· Maintain inventory of all supplies used to provide structured programming to the participants of the Senior Day Program.
· Maintain participant and staff confidentiality while treating them with dignity and respect to maintain a secure environment for all.
Education Requirements:
· Minimum AA degree in nursing, health services or human services and
· Certified Personal Care Aide
Licenses:
· Valid NYS driver’s license
Experience:
· Two-year experience in health services or human services, with one-year experience in program coordination.
Work Schedule:
· Primarily Monday-Friday during regular business hours but must be flexible. May require after hours and weekends. Usual shift is 7.5 hours.
